California Power of Attorney for Health Care and Health Care Instruction Form
(also called an Advance Health Care Directive or AHCD)
By completing this form you can do either or both of the following two things:
In Part 1 (Naming Your Agent), you may appoint an agent who will have legal authority to make health care decisions for you in the future, if and when you no longer have the mental capacity to make your own decisions. Your doctor will determine when you are unable to make these decisions for yourself.
In Part 2 (Health Care Instructions), you may state your instructions regarding future health care, including your desire to receive or not receive life-sustaining treatment and under what conditions. Your lawful instructions must be honored by your doctor and other health care providers, as well as your agent (if you have appointed one).
You may revoke a California Power of Attorney for Health Care and Health Care Instruction Form at any time.
